Prior to your consultation, you will receive an intake questionnaire. In addition to basic information, there is also detailed information we request concerning your assets. We ask that you take your time and fill out the form to the best of your ability.
While it's very important to have your family information, beneficiary information, and your intent, it's also equally important to have information regarding your assets, where those assets are located, and the approximate value of those assets. That provides us with the best opportunity to give you your estate planning options and make sure that your estate is distributed in accordance with your wishes.
